Report an Issue to the IT Service Desk (Only if Issue or Request is NOT in the Service Catalog)

What Is It?

An IT issue is when something unexpected messes with your computer or tech stuff. It could be a virus, a software problem, a hacker causing trouble, or even just a glitch that messes things up. Basically, it's when your tech isn't doing what it's supposed to do, and it can be a real headache. This link will allow you to create a service ticket that will go directly to the USI Service Desk. The Service Desk Team will evaluate your ticket and assign to the correct team.

If you search for your issue in the service catalog, you will get a customized form, and your ticket will be immediately directed to the team that can help with your problem. We highly recommend searching for your issue in the Service Catalog and Knowledge Base. It could  save you a tremendous amount of time.

Use the red Report an Issue button on this screen to let us know what is going on. We will process your ticket as quickly as we can and get you back up and running with as little downtime as possible.
