What Is It?

Use this service to report that you have lost access to your USI or any other related accounts. If you are unable to login to MyUSI or your email account, please fill out this form, and we will help you get your access back.

Who is Eligible to Use It?

Students, faculty or staff who no longer have access to any USI account.

How Do I Use It?

Use the red Help! I'm Locked Out! button at the top of this page to submit a ticket to the USI Service Desk. Make sure to include a phone number and email address that you have access to.

How Much Does It Cost?

There is no fee associated with this service.
